First off this is for concrete feedback on the UI issues with MediaAttach, any complaints with out concrete suggestions will be removed, this is not a place to complain that you don't like the UI, this is a place to tell us what you want/expect out of the UI.

I don't know the interface of MediaAttach, but I can tell the best attachment interface I have seen so far is used by InvisionPowerBoard version 3.
It is so slick that it even allows to select multiple files to upload from your computer.
So, I'd suggest your UI takes some ideas there, if not done already. -

I think that before any UI discussion takes place, it should be determined what the target audience of MediaAttach is. I feel a lot of confusion on the usage is due to it being the wrong tool for the job for many cases. I can pound screws in with my hammer but it's a bad idea. That doesn't make my hammer a useless tool with a bad UI, it just has to be used correctly for the correct task.
Since MediaAttach is a "larger" module, perhaps there should be an outline written on its intended usage, and maybe some usage rails (do this, do this, do this, you're done).
