Example of using attachment upload form fields in Formicula

Can someone share example templates for Formicula that include a file upload to be attached to the email sent to the admin? I'm having difficulty getting that feature to work, and neither of the examples included with the module have a file upload. I can't find any documentation regarding that feature, either.
