  I can't reproduce this. I've only got a clean install to look at but can't see this 'everywhere'. Can you be more specific.
  
  Further I cannot find any reference to this image file anywhere in the core distro. This must be generated by a 3rd party module.

  Yes it doesn't come from the core sorry, after some search it's an error with the last SVN of MultiHook.

  That would make sense> To fix this change the pnimg in the template mh_dynamic_hiddenform.html. Something like the following will fix it. Choose your Ajax image from images/ajax.
