Lets say I want to create a form like this
Name : [BOX TO FILL IN NAME]
Address : [BOX TO FILL IN ADDRESS]
AGE : [BOX TO FILL IN AGE]
And so on, and after that, it will tabulate everything into a nice file which I can look at the info easily.
I've searched and it seems FormExpress is good, however, when I tried to use it, it seems it doesn't work for 0.750!
Can anyone introduce me a good form maker?

There is a version that will work with 750, but it doesn't store the data with out some other work. I'd use Pagesetter.

Try reading the Manual first, it is well written and quite detailed.

Your best bet would be to visit Jorn's site and post on his forums. He comes 'round here from time to time, but you're more likely to the help you need over there.

1. I don't have the manual in front of me, but I'm not sure that the process can be much less simplified. Follow the tutorial.
2. As Spire says, it's still best to ask questions at Jorn's site. More focus on Pagesetter, and as such more specific help on it there than you'll find here.
3. When you hit something in the manual you don't understand, ask about that part.
Regardless of using PS or FE, creating a form through any such program is going to have a higher degree of complexity due to the nature of the beast.

To be honest, whenever I need a form of any sort, I just make a form from scratch with HTML or PHP and place it in my PN directory and call the form outside the PN structure in a pop-up window, sometimes including the page header to resemble the site. Then I make a PHP output page and place that in the site as well, technically exterior to the whole site structure, but designed to look like the rest of the site.
You do this one time and you just kep re-using it with minor mods every time thereafter. This assumes you are able to create and process forms with PHP. -

Yeah but FE is a pain in the butt to work with and if you forget a field adding it is painfull. Pagesetter is by far the best choice for well, anything-the ability to define all fields will make the form powerful. Not to mention eith Pagesetter you can control the way the data is displayed-somehting that is a pain in the neck with all other methods...
