What I do, is take a current theme, and modify it... since your starting from scrach use a XTE theme... you will thank me later...
Install postNuke, then go to Admin->Modules and initialize/activate Xanthia.
Then go into Admin->Xanthia, and turn on all the themes...
Then view each one, and find one that has a simular layout... Dont worry about the look of it... just pay attention to how the tables work... Are they variable, do they have any "special" spacing, etc...
Pick one like your site... then go into your file system and copy the theme in question... Then go through the theme and look for invalid paths...
Finally view it and see if it works... if you have problems you missed a path... so go back and look...
lastly, edit each element moving towards your goal...

ohh one more thing...
Go to Admin->Xanthia->Configure Xanthia
and check "Check for updated version of templates", and uncheck "store templates in database"
this should turn off the cache. I think you could turn on Force templates recompilation too, but I never did... When your done with the look and feel, turn this back.
Make sure that you understand CSS, and try not to edit anything not under the /themes/ directory cause these changes wont work when you upgrade...
